وَسَهِرَتْ فِي مَدَارَاتِهِ،
And she kept vigil in attending to him,
مَدَارَاتِهِ — attending to him. This is a plural action-noun, 'the tendings-of / managing', with the 'him' pronoun on its end, sitting in the genitive after the 'in' before it. So action, object, and case all combine in one word. The -hi keeps pointing back to the son she cared for.
