وَرَسْمِهِ — and his form. This is the bound wa- 'and' plus a noun, 'and his form', carrying the attached -hi 'his'. Coordinated by wa- onto the previous 'ala phrase, it shares that preposition's possessed ending, so 'upon his name and his form' runs as one governed pair.
