ثُمَّ الغَرَامَ وَهُوَ الْحُبُّ الْمُلَازِمُ لِلْقَلْبِ
Then comes ardent love, the love that clings to the heart.
الْحبّ — the love. A definite noun 'the love' serving as the predicate of the clarifying clause, in the subject (nominative) form. It restates the stage as a kind of love, and the participle after it specifies which kind.
