He would not accept a fee for treatment from the poor and middle-class people.
يَأخُذُ — he takes. A present-tense verb 'takes' with its subject 'he' built in; under the earlier 'was' it reads as a repeated, customary action. The present tense is what gives the 'he would take' habitual sense.
