فَفُتِّتَ — so it was crumbled. The fa- delivers the result -- 'so it was crumbled' -- on a PASSIVE verb: the bread UNDERGOES the crumbling, with no doer named. Arabic builds the passive by shifting the inner vowels, not by adding 'was', so the form looks close to its active twin but flips the role.
