وَتَحْتَ رَأْسِهِ مِرْفَقَةٌ مِنْ أَدَمٍ،
And beneath his head was a leather cushion.
مِرْفَقَةٌ — a cushion. An indefinite noun 'a cushion' that is the delayed subject of the place-frame ('under his head was a cushion'); its plain subject ending and indefinite tanwin introduce the item. There is no 'was' word, just the place-phrase plus this subject.
