قَالَتْ نَعَمْ فَخَرَّتْ مَغْشِيًا عَلَيْهَا،
She said, 'Yes,' then she collapsed, fainting upon her.
مَغْشِيًا — fainting. This describes the state she was in as she fell ('fainting'), a circumstance-word that takes the accusative ending for exactly that role. It tells you HOW the collapse happened, layering a condition onto the verb.
