وَأَشَدُّ مِنْ ذَلِكَ أَنْ يَقَعَ السُرُورَ بِمَا هُوَ عُقُوبَةٌ،
Even more severe than that is taking pleasure in what is a punishment.
عُقُوبَةٌ — a punishment. An indefinite noun, no 'the', naming a punishment, the predicate said of the pronoun 'it'. Carrying the buzzing indefinite ending, it states 'it is a punishment'. So it delivers the sting: the very thing rejoiced over is, in truth, a chastisement.

So when a punishment befalls to test a sin,
عُقُوبَةٌ — a punishment. An indefinite noun standing as the subject of the verb just before it, so it carries the plain subject ending. It is what does the befalling. Left indefinite, it means 'some punishment', any instance, fitting the general 'whenever' frame the clause is building.
