These obstacles stand between the heart and its understanding.
هَذِهِ — these. A demonstrative meaning 'these', feminine to match the plural of things, used as the subject pointing back to the obstacles just listed. It is definite by nature and stands in the subject case as the topic of the sentence.

If the servant wants to repair his heart, let him cut these obstacles,
هَذِهِ — these. A pointing word, 'these', used for nearby or just-mentioned plural things. It agrees with the noun it introduces and, being a demonstrative, it also lends that noun definiteness, the way English 'these' already singles things out.
