وَالْحَرْبُ سِجَالٌ،
And war is fierce fighting.
وَالْحَرْبُ — and the war. A connector 'and' prefixed to a noun 'war' made definite by its built-in 'the', so one word means 'and the war'. It stands as the topic of a noun-based 'X is Y' statement whose 'is' is left unspoken.

الْحَرْبُ — the struggle. A definite noun, the subject of the clause that follows. In this passage 'the struggle' is the figurative back-and-forth between sickness and strength, so 'al-' points to that ongoing inner contest rather than a literal war.
