إِذا لم يعد صبا بلقيا حَبِيبه
If it does not return to being youthful upon meeting its beloved.
صِبًا — to being youthful. A noun 'youthful ardor / fresh longing' in the accusative ('-an' tanwin) — here the predicate of 'become', 'become [again] young with passion'. The accusative marks the state it would return to.

And there is a third opinion by Muqatil, who said that if He wills, He can return him from old age to youth, from youth to childhood, and to the sperm-drop.
الصِّبَا — childhood. This noun 'childhood' is in the genitive forced by the preposition before it. As a noun ending in a long vowel it shows no audible case change, but it fills the genitive slot as the endpoint of the transition.
