وَتَفْعَلُ مَا تَحْتَاجُ إِلَيْهِ مِنَ الرَّوَثِ وَالْبَوْلِ أَيَّ وَقْتٍ اتَّفَقَ
And they perform what they need like defecation and urination at any time.
أَيَّ — any. A word for 'any, whichever' that opens a possessive pair with the next noun; here in the accusative, working adverbially as 'at any...'.

So consider which of the two conditions you want to be in.
أَيَّ — which. A question word meaning 'which', the first half of an 'of' pairing with the noun after it, 'which of the two states'. It is the object of the following verb so it takes the object case and owns the pairing that follows.

أَيَّ — any. This is an exclamatory intensifier, 'what a...!', set in the accusative and fronting a possessive pairing with the noun after it. It is not a question word here; it heaps emphasis on the stumble, turning a plain fall into an outcry over how bad it was.
