ثُمَّ أَقْبَلَ عَلَى ثَدْيِهَا يَمَصُّهُ
Then he leaned toward her breast and sucked it.
ثَدْيِهَا — her breast. The breast he turned to, governed by 'toward' and so in the genitive, with '-ha' (her) attached pointing back to the mother. The suffix marks whose breast, and the case marks it as governed by the preposition.
