Abu Musa al-Ash'ari reported that he performed ablution in his house, then he went out.
تَوَضَّأَ — performed ablution. A past verb of the reflexive pattern (an action done to oneself), here 'he performed his own ablution'. The shape of the verb itself signals the do-it-to-yourself sense, with a built-in 'he'.

Then he performed ablution, and then I rose to him.
فَتَوَضَّأَ — then he performed ablution. The fa- ('then') chains this to the prior action; the verb is the reflexive-pattern past 'he performed ablution on himself', its shape signalling the action is done to oneself. A built-in 'he' is the doer.
