في الآخرة يظهر ميزانٌ لا يخطئ ولا يميل،
In the Hereafter, a scale will appear that neither errs nor sways,
يُخْطِئُ — errs. Present 'errs, gets it wrong', subject 'it' inside; form-IV. With the noun indefinite, it works as 'that errs'.

And know that what missed you was never going to reach you.
أَخْطَأَكَ — missed you. A Form IV verb, 'failed to hit, missed', subject 'it' inside and '-ka' the object 'you'. It describes 'that which': 'what missed you', the thing that did not reach you.

He made a mistake out of overwhelming joy.
أَخْطَأَ — He made a mistake. This is a past-tense verb with its 'he' subject inside, meaning he erred/misspoke. It states plainly what the preceding outburst was, a slip, and the cause is given by the prepositional phrase that follows.
