وَإِنَّا نُقَلِّبُ أَفْئِدَتَهُمْ وَأَبْصَارَهُمْ كَمَّا لَمْ يُؤْمِنُوا بِهِ أَوَّلَ مَرَّةٍ
And indeed We will cause their hearts and their sights to turn just as they did not believe in it the first time.
أَوَّلَ — first. An ordinal 'first' in the object case, modifying the time-noun after it to mean 'the first time'. As part of an adverbial time phrase it tells when the disbelief happened.

أَوَّلَ — the first. This noun is the subject of the 'indeed' statement, so it takes the accusative that 'indeed' imposes on its subject. It heads a possessive pairing with 'the people' next, 'the first of the people', singling out who comes first at judgment.
